If you own your home (have a mortgage/deed in your name) please email Bridget Baker the name and email address of the parent/legal guardian whose name is listed on the mortgage/deed *If more than one parent/legal guardian is listed on the lease, only one parent/legal guardian name and email address is required.
If you Rent (have a signed lease agreement with your name listed as the lessee) please email Bridget Baker the name and email address of the parent/legal guardian whose name is listed as the lessee on the lease *If more than one parent/legal guardian is listed as the lessee on the lease, only one parent/legal guardian's name and email address is required.
If you live at a residence owned or rented by someone other than the student(s) parent(s)/legal guardian(s) please email the name and email address of the owner or lessee of the residence as well as the name and email address of one of the parents/legal guardians to Bridget Baker.
*Please note the residency statement does need to be notarized. Once you have completed the Docusign you can either print off the completed residency statement and bring it into central office, 245 So. Franklin Street (door # 8 in the back of the building) anytime between 8:00am-4:00pm Monday-Friday to have it notarized or print it off and have it notarized by another notary. The original notarized form must be returned to central office (Door #8) before you can move forward with the application process.
