Change of address (Moved within Holbrook)

The parent/legal guardian has two options to complete the change of address:

OPTION 1:  Come into Central Office (Door #8 in the back of the building) anytime between 8:00am-4:00pm Monday-Friday *Summer Days/hours – Monday-Thursday 8:00am-3:00pm

OPTION 2:  Have a DocuSign link emailed to redo the residency requirement.  Please email Bridget Baker at bbaker@holbrook.k12.ma.us the names and email addresses of the parent/guardian the link should be sent to.

*Please note the parent/legal guardian will need to come in to have their signature notarized on the residency statement once it has been completed.

RESIDENCY REQUIREMENT

In order to attend the Holbrook Public Schools, parents, legal guardians or students 18 years or older must first prove they reside in the Town of Holbrook by completing the residency requirement.

Parents, legal guardians or students 18 years or age or older must redo the residency requirement by submitting at least one document from each of the below three columns.  The same current Holbrook residential street address must be present on all three documents as evidence of residency.

Column A: 

·        Copy of deed or most recent mortgage statement 

·        Signed copy of lease agreement (including EBHA and HUD leases) 

·        Notarized signed legal affidavit from the landlord affirming tenancy indicating the terms of the lease and all authorized tenants §

·        Signed Section 8 lease agreement 

·        Signed Purchase and Sale Agreement OR Insurance Binder 

·        Most recent Town of Holbrook Tax Bill

Column B: 

·        A utility bill OR utility work order dated within the past 60 days; including: gas bill, oil bill, electric bill, home telephone bill (landline only) or cable bill

 

Column C: 

·        Valid Driver’s License 

·        Valid Massachusetts Identification Card 

·        Valid Passport

The person whose name is on the lease or mortgage is the individual who must complete the online registration and have their signature notarized on the Holbrook Public Schools Residency Statement.   The original notarized form must be returned to Central Office, Door #8 in the back of the building.  Scans or copies will not be accepted.

PENALTIES

Any student and/or family suspected of residing outside the Town of Holbrook while attending Holbrook Public Schools may be subject to a residency investigation.

If a student is found to be in violation of this policy, they will be withdrawn from Holbrook Public Schools immediately. In addition, the district reserves the right to impose further penalties on families found in violation, including but not limited to:

  • Legal action;
  • A financial assessment based on the cost of educational services provided during the period of non-residency, not to exceed the average per-pupil expenditure.

Holbrook Public Schools reserves the right to request various forms of documentation and to investigate a student’s actual place of residence. Because residency may change during the school year, the district may continue to verify residency after enrollment has begun. The district may also initiate investigations based on both anonymous and identified reports.

Residency investigations may be conducted in situations including, but not limited to:

  • Submission of incomplete, inconsistent, or suspicious proof of address;
  • Reports of potential residency violations from anonymous or identified sources;
  • Returned mail from the school district due to an invalid address.
