Step 1    Complete the residency requirement

Residency Verification

The first step is to complete the residency requirement.  You can either come into Central Office (Door #8 in the back of the building) with the required documents between 8:00am-4:00pm Monday-Friday or a Docusign link can be sent to your email address to begin this requirement. Please email Bridget Baker at bbaker@holbrook.k12.ma.us indicating which of the below applies:

Own your home (have a mortgage/deed in your name). If yes, please email the name and email address of the parent/legal guardian whose name is listed on the mortgage/deed. *If more than one parent/legal guardian is listed on the mortgage/deed, only one name and email address is required.

Rent (have a signed lease agreement with your name listed as the lessee).  If yes, please email me the name and email address of the parent/legal guardian whose name is listed on the lease. *If more than one parent/legal guardian is listed on the lease, only one parent/legal guardian name and email address is required.

Do you live at a residence owned by someone other than the student(s) parent(s)/legal guardian(s)If yes, please email the name and email address of the owner of the residence as well as the name and email address of one of the parents/legal guardians.

 

Step 2

Once the residency requirement has been completed the school secretary will contact you to start the enrollment process.
