Please feel free to contact the nurse during school hours of 7:30-2:15 with any questions or concerns. Please notify the school nurse if your child is unable to attend school. Messages may be left on the private nurse line and voicemails will be checked throughout the day.
Daily screening for COVID-19:
Below is the full list of symptoms for which caregivers should monitor their children, and staff should monitor themselves:
? Fever (100.0° Fahrenheit or higher), chills, or shaking chills (CDC has lowered the temperature from 100.4 to 100.0)
? Cough (not due to other known cause, such as chronic cough)
? Difficulty breathing or shortness of breath
? New loss of taste or smell
? Sore throat
? Headache when in combination with other symptoms
? Muscle aches or body aches
? Nausea, vomiting, or diarrhea
? Fatigue, when in combination with other symptoms
? Nasal congestion or runny nose (not due to other known causes, such as allergies when in combination with other symptoms.
If staff or students have any of these symptoms, they should get a test for active COVID-19 infection prior to returning to school. If an individual is not tested, the individual must remain home in self-isolation for 10 days from start of symptoms, then return once symptom free for 24 hours without the use of fever reducing medications.

General MA Immunization Requirements:
The school nurse is responsible for ensuring that all students have documentation of all MA school requirements. Documents can be mailed, dropped off, emailed to ifroio@holbrook.k12.ma.us, or faxed to 781-815-1627.
New 2020-2021 immunization requirement for 7th grade:
All incoming 7th graders will need to provide documentation of a Tdap and a meningococcal conjugate vaccine. Please consult with your child's pediatrician to ensure these have been given and provide the school nurse with documentation. Click here for more information on this new requirement.
New 2020-2021 immunization requirement for 11th grade:
All incoming 11th graders will need to provide documentation of having 1 booster dose of meningococcal conjugate vaccine (Men ACWY) received on or after 16 years of age. Please consult with your child's pediatrician to ensure these have been given and provide the school nurse with documentation. Click here for more information on the new requirement.
FLU VACCINE
Updated 8/20/2020: MA FLU Vaccine: The MA DPH has added the seasonal flu vaccine to the list of required immunizations for all grades K-12 students. Students should receive the seasonal flu vaccine and provide proof of the vaccine to the school health office by December 31, 2020. All students attending the HM-HS, even in the hybrid or remote model, must comply with this directive The only exceptions are medical or religious exemptions and a letter must be on file in the health office if one of these exemptions are being utilized. Medications:
If your HMHS student will require medication during the school day, you can call the school and make an appointment to visit with the nurse. The nurse phone number is 781-767-4616 ext 3
Please remember to bring:
1. The medication in its original prescription bottle
2. A medication order from the MD or NP. It may also be faxed to 781-815-1627.
3. Yourself! A parent or guardian needs to sign the parental consent form
